How to Apply

Please follow both steps below. You must submit items to both UW-Madison and UW-Stevens Point. Pay close attention to the requirements for transcripts and GRE score.

Your application will be considered by the joint Au.D. Admissions Committee. You will be notified of your admission status during the spring semester. It is your responsibility to notify the Au.D. admissions committee of any changes in contact information.

Step 1: UW-Stevens Point Application

  1. Application Fee. You must submit a nonrefundable application fee of $56 ($60 for International applicants) with the completed Admission to Graduate Study form. The payment should be made by personal check or money order, drawn on a U.S. bank. Please do not send cash. The personal check or money order should be made out to 'UW-Stevens Point'. (Please write your birth date on your check, so we can match it to your application.) An additional $50 express courier fee may be charged to International applicants.
  2. UW-Stevens Point Admission to Graduate Study Form. Complete and print the entire form, and submit it to the UW-Stevens Point Admissions Office at the address above.

International applicants should complete and submit the UW-Stevens Point International Graduate Application

Step 2: Au.D. Program Application

Submit the following application materials by February 1st to:

  1. Au.D. Application Form. Complete and sign this program application, print it, and submit it to the Au.D. Admissions Committee.

    On your application, you must specify a preferred home campus. This refers to which university (UW-Madison or UW-Stevens Point) you prefer to be located.

  2. Transcript. Submit an official transcript of all postsecondary institutions you attended, including any graduate work. The transcript may come directly from the institution or from the applicant, but must be in a sealed envelope.

    If your native language is not English, or your undergraduate instruction was not in English, send your international academic records in the original language accompanied by an official English translation. These documents must contain the official seal/stamp and official signatures of the issuing institution. You must also send a set of descriptions of the courses listed on the transcripts and/or certificates.

  3. Statement of Professional Intent. Your statement of professional intent should state your professional interests and goals, why you want to enter the profession, and why you think you will be successful in this profession. Please sign this letter.

  4. Letters of Recommendation. Submit three (3) recommendations, at least two of which should come from faculty members who have knowledge of your academic performance. The third recommendation can come from a clinical supervisor, employer, or other individuals who have demonstrable knowledge of your academic potential and likelihood for success in graduate school.

    Please use the Recommendation Form for your letters. Any additional documentation should address academic performance, motivation, personality and potential. All recommendation materials should be submitted in a sealed envelope with the recommender’s signature across the flap.

  5. GRE & TOEFL. All domestic and international applicants must submit a GRE score. If your native language is not English, or your undergraduate instruction was not in English, you must also provide official scores from the Test of English as a Foreign Language (TOEFL).

    GRE and TOEFL scores should be sent to UW-Madison directly from ETS (use institution code 1846). The code for the Audiology department is 0602.
