Send to the Department:
- Reasons
for Graduate study (if not included in with the online application).
- List
of awards, honors, and/or publications (if not included with the online
application).
- Two
copies of Official Transcripts
of all post-secondary institutions you attended, they can come directly
from the institution or from applicant, but must be in sealed envelope.
(One will be sent later to Graduate School if
Department recommends admission)
- Three
letters of reference/evaluation (Evaluation form is available online or
letterhead can also be used)
- Test
scores for standardized tests: The GRE is required for consideration of
the application for both domestic and international applicants. Every applicant whose native language is not
English, or whose undergraduate instruction was not in English, must
provide official scores from the Test of English as a Foreign Language
(TOEFL). GRE and TOEFL should be sent electronically from ETS.
Use institution code 1846 to request test scores from ETS to be sent to
UW-Madison. The code for the department is 0602 for Audiology
and 0620 for Speech and Language. It is your responsibility to make sure
that the test results are sent to the University, so it is important that
you use the correct codes. Find information for GRE at http://www.gre.org
and TOEFL at http://www.toefl.org.

USD non-refundable application fee. If you did not pay by credit card
online, you must send a money order or check drawn on a U.S. bank, made
out to '
University of Wisconsin-Madison'.
(Please write your birth date on your check, so we can match it to your
application.) The Department will not make a recommendation
for admission until we receive your application fee.

Please do not send transcripts, reasons for graduate
study, letters of recommendations or any other departmental documentation to
the Graduate School. These
materials will not be forwarded to your department/program. The Graduate School no longer accepts paper
applications, you must apply online only.
International applicants: Please do not send financial documents until
requested by the Graduate School. This information will be requested upon a positive
admission recommendation from the department/program.
